This Waiver and Release of Liability Form (“Waiver”) is between Matthew McDaniel

doing business under the name McDaniel Muscle Academy (“Trainer”) and the undersigned,

physical training client (“Client”), in connection with Client’s voluntary participation in any

and all physical training exercises whether at Trainer’s personal residence, outdoors, or within other facilities.
